By accessing or using the Biogenomics provider portal ("Service"), you agree to be bound by these Terms of Service. If you do not agree, you may not use the Service.

The Service is restricted to licensed medical providers with an active NPI number and valid state license. Registration requires verification against the NPPES registry.

FOR RESEARCH USE ONLY. Products sold through the Service are intended for research purposes only and are not intended to diagnose, treat, cure, or prevent any disease.

You are responsible for maintaining the confidentiality of your account credentials and for all activity under your account.

Biogenomics may update these Terms at any time. Continued use of the Service after changes constitutes acceptance of the updated Terms.
